What is the new process for NDIS plan reassessments

Support coordinators can't process the plan reassessment request you lodged the usual way because the new NDIS laws came into effect on 27 August 2026.

Under these laws, a participant can't give consent for another person, such as a provider, to request a plan reassessment on their behalf. The participant, their plan nominee, child representative, or legal guardian must complete it and include recent evidence (usually within 4 months) showing the change.

NDIA has released a new form for this purpose: the Plan Reassessment Form.

A Plan Reassessment Request Form is the official NDIS form used when a participant needs a new NDIS plan because their support needs have significantly and ongoingly changed.

The details to be completed in this new form are as follows -

Overview

A Plan Reassessment Request Form is required when your current NDIS plan no longer meets your disability support needs due to a significant and ongoing change in areas such as:

This form is not used for minor or short‑term changes (those use a Plan Variation Request Form). National Disability Insurance Agency

  • Your disability support needs have changed substantially and permanently.

  • You need more, less, or different NDIS supports.

Who Can Submit the form

Only the following can lodge the form:

  • The participant

  • A plan nominee

  • A child representative

  • A legal guardian

Providers cannot submit it, even with consent.

Evidence Required

The form must include recent evidence (typically within 4 months) showing the significant change. This may include:

How to return the form once it is completed

  • You can return this form by submitting a new inquiry through the NDIS Service Hub. Make sure you upload the completed form to the inquiry

  • Sending it via post to: NDIA, GPO Box 700, Canberra ACT 2601

  • Giving it in person by visiting a local area coordinator, early childhood partner, or NDIS office in your area.

What Happens After Submission

  • The NDIA has 90 days to decide whether to reassess your plan, vary it, or leave it unchanged.

  • If the NDIS doesn't decide within 90 days, the request is considered declined.

  • You can keep using your current plan while you wait.

A Plan Reassessment Request Form ensures the NDIS can update your plan, so it continues to meet your disability‑related support needs when major changes occur.
