Hypothetical Scenario –

Vanessa is 52 and lives alone. She has gone through a surgery on her shoulder recently, which affects her health and her mobility. There are times when she is in pain and finds it difficult to do household tasks.

The Physiotherapist from the Local Health District assisted Vanessa with some light exercises and referred her to the ComPacks SASH Program.

The ComPacks SASH Case Manager visited Vanessa to discuss what community services were needed to support her at home. It was agreed that Vanessa would need intermittent support with cleaning the house, preparing meals and transport to shopping.

After 6 weeks, Vanessa’s symptoms have settled and she no longer requires community supports through the program.

The Case Manager has linked Vanessa to a gentle exercise class one day per week
