How to Apply for becoming an NDIS participant

A person who has some sort of disability and may wish to apply for NDIS must have gone through the elibility checklist on the NDIS website. Once you see yourself being able to answer all eligibility questions in yes, you can download and complete the below two forms to lodge your application for entering the scheme (Source - NDIS website).

2. Supporting Evidence Form - Weblink here

Complete the Form No. 1 and work with your GP or health professional to complete evidence form No. 2 and send it via -

1. Email to - NAT@ndis.gov.au

2. Mail to: GPO Box 700, Canberra, ACT 2601

3. Drop In person: take it to your local NDIA office
