NDIS Support Coordination

What is the role of Support Coordinator in NDIS?

Support Coordinators plays a key role in supporting ndis participant to navigate the complex ndis support market and build a participant's knowledge and capacity to learn about ndis.

 

A good support coordinator will have enormous knowledge about ndis, mainstream and other community supports and will have a very good understanding of working with people with disabilities.

NDIS Planning and implementation of support being one of the key focus of a support coordinator, if done correctly can help you achieve the most out of your NDIS plan.

 

What is included in the support coordinator's role?

A support coordinator helps participants in searching, coordinating, and negotiating with service providers of choice. 

 

A support coordinator constantly communicates with participants to ensure progress and effective delivery of services as per the participant's goals.  

A good support coordinator will -

  • Be Available and communicative

  • Help you understand the NDIS plan

  • Assist in successfully implementing the plan and support your NDIS plan reviews

  • Guiding and supporting in understanding NDIS portal

  • Assisting to navigate the system and contact providers of choice

  • Discuss and help to understand Service Agreements with providers

  • Assist in building capacity and independence in order to coordinate services on your own if required

What is not included in the support coordinator's role?

Support Coordinators cannot provide services such as Transport, Plan Management, Rostering,
Advocacy, Disability Support, Case Management, and  Chasing of financials. 

When providing you with Support Coordination, the same organization can also provide you with other supports.  While providing you with other supports besides support coordination, it is important to inform you of any potential conflict of interest arising and that you are also given the opportunity and option to choose a service provider of your choice.

What are the different levels of Support Coordination?

Level 1. Support Connection -

Support Connection is provided to assist a participant in understanding the aspects of the plan, assisting in ongoing management of support, and answer questions as they arise.

Level 2. Support Coordination - 

Qualified and/or experienced person assisting to strengthen a participant’s ability to design and build their supports. The emphasis is to link with the broader systems of support across a complex service delivery environment.

 

It is support delivered with a particular focus to direct participants' lives, not just their services. This may include resolving points of crisis, and developing resilience in the participant's network.

Level 3. Specialist Coordination -

Qualified and/or experienced practitioner providing time-limited, outcomes-focused support to meet the individual needs of the participant’s circumstances.

 

This is provided with a focus to reduce complexity in the participant’s support environment in the context of broader systems of support, whilst also assisting the participant to connect with NDIS supports.

 

This support also assists to negotiate solutions with multiple stakeholders and build the capacity and resilience of a participant.
